ReumatoÔde artritis is een zogenaamde auto-immuunziekte. Dat betekent dat uw lichaam antistoffen maakt tegen het eigen lichaam, in dit geval tegen het weefsel aan de binnenkant van het gewricht. De antistoffen veroorzaken een ontsteking die de gewrichten kan beschadigen. Waarom iemand reumatoÔde artritis krijgt, is niet bekend. In tegenstelling tot wat vaak gedacht wordt, speelt erfelijkheid nauwelijks een rol. Medicijnen zoals ibuprofen, diclofenac of naproxen, kunnen de pijn verminderen en de ontsteking remmen maar kunnen tot uitslag en maagklachten leiden.

Researchers Uncover Higher Prevalence of Periodontal Disease in Rheumatoid Arthritis Patients

Study Published in the Journal of Periodontology Suggests Impaired Oral Hygiene May Only Be Part of the Connection

Over 1.3 million Americans suffer from rheumatoid arthritis (RA), a chronic, inflammatory disease of the joints. RA is a disabling condition, and can lead to long-term joint damage resulting in persistent pain and loss of function in affected areas. A recent study published in the June issue of the Journal of Periodontology, the official publication of the American Academy of Periodontology (AAP), uncovered yet another potential side effect of RA. Researchers in Berlin, Germany discovered that patients with RA have a higher incidence of periodontal disease compared to healthy controls.

For some patients, adverse RA symptoms may affect manual dexterity, which can make one’s daily routine quite difficult. One area that may be affected is oral hygiene which can ultimately lead to periodontal disease. However, these research findings indicate that poor oral hygiene alone did not account for the association between RA and gum disease, suggesting that other factors may play a role as well. The study examined the oral health of 57 RA patients and 52 healthy controls. To determine oral hygiene status, each participant underwent a comprehensive oral examination including an assessment of plaque accumulation and gingival inflammation, both indicators of oral hygiene. Probing pocket depth and clinical attachment loss, two markers of periodontal disease, were also measured. Researchers used questionnaires to gauge the subjects’ risk factors for periodontal disease. The study findings indicated that RA patients were nearly eight times more likely to have periodontal disease compared to the control subjects. These findings accounted for demographic and lifestyle characteristics such as age, gender, education and tobacco use. Researchers then examined the extent to which poor oral hygiene was connected to the increased occurrence of gum disease in RA patients. The results showed that while oral hygiene was markedly a factor, it did not fully explain the association between the two diseases, suggesting that there may be other parameters responsible for the increased prevalence of gum disease in RA sufferers.

“With results suggesting that rheumatoid arthritis is associated with periodontal disease, it is easy to assume that an RA sufferer is perhaps unable to properly care for his or her teeth and gums due to the debilitating nature of the disease,” says Dr. Kenneth Kornman, editor of the Journal of the Periodontology. “However, this study implies that there are other potential factors involved. For instance, both RA and gum disease are systemic inflammatory disorders which may explain the connection between the two. Inflammation is already thought to link periodontal disease with other conditions such as cardiovascular disease and diabetes. We look forward to future research that may reveal the biological mechanisms that link these two important diseases.”  In an effort to best maintain oral health, RA patients are encouraged to brush and floss on a regular basis and see a dental professional twice a year. If gum disease develops, consulting a periodontist is an effective way to determine the most appropriate course of treatment. According to Dr. Susan Karabin, President of the AAP, maintaining the complete health of RA patients should be a collaborative effort. “It is critical that dental professionals and medical professionals work together when treating a patient living with rheumatoid arthritis. This partnership will assure that both the oral and overall health of these patients is paramount.”

Kerry en artritis

Brachten we deze week al het nieuws dat kerrie de ziekte Alzheimer en mentale achteruitgaan kan afremmen is er nu weer een kerrie nieuwtje. De stof  turmeric kan ook reuma, artritis en osteoporosis voorkomen. Ze denken dat puur het eten van extra kerrie niet zal helpen maar dat er wel een medicijn op basis van deze stof ontwikkeld kan worden die misschien ook kan helpen bij onstekingsgerelateerde aandoeningen zoals astma, IBD en MS.

Verschijnselen

Er ontstaat pijn, stijfheid en zwelling in de gewrichten van bijvoorbeeld de handen, polsen, enkels of voeten. Meestal zijn de verschijnselen zowel links als rechts in dezelfde gewrichten aanwezig. Door de ontsteking wordt het gewricht dik en gevoelig, soms ook warm. Een ontstoken gewricht doet zonder te bewegen of te belasten al pijn. De stijfheid is 's ochtends het ergst en kan een tot enkele uren duren.

Nieuwe hoop voor artritis patiŽnten

Corticosteroiden verpakt in doelzoekende liposomen
Corticosteroiden zijn krachtige ontstekingsremmers, maar berucht om hun
bijwerkingen. Om de effectiviteit van corticosteroiden te verhogen en
bijwerkingen te verminderen, heeft een team UIPSonderzoekers, onder
leiding van Gerben Koning en Gert Storm, de corticosteroiden ingekapseld
in liposomen.

De liposomen werden bovendien voorzien van peptiden die ervoor zorgen dat
liposomen specifiek binden aan de bloedvaten in ontstoken weefsels. Op die
manier wordt meer van de dosis afgeleverd op de plek van de ontsteking en
minder op plaatsen waar de corticosteroiden bijwerkingen zouden kunnen
veroorzaken. De onderzoekers hebben nu aangetoond dat diermodellen
behandeld met deze bewerkte liposomen veel beter en langduriger reageerden op de therapie dan wanneer dezelfde dosis werd toegediend op de conventionele manier. De onderzoekers publiceerden hun bevindingen in het gerenommeerde wetenschappelijk tijdschrift Arthritis & Rheumatism. In de buitenlandse pers heeft het artikel al geleid tot extra publiciteit.

Komt meer voor bij vrouwen dan mannen

De ziekte komt drie keer meer voor bij vrouwen dan bij mannen. De ziekte kan op alle leeftijden optreden maar het meest frequent tussen veertig en vijftig, ongeveer het ogenblik waarop ook de menopauze begint.
De juiste oorzaak is onbekend, maar waarschijnlijk is bij de ziekte het afweersysteem van het lichaam ontregeld waarbij het lichaam zijn eigen weefsels aanvalt en er ontstekingen in diverse gewrichten ontstaan. Vooral de kleinere gewrichten in de handen en de voeten worden getroffen. Het gewrichtskapsel raakt geleidelijk aan ontstoken en opgezwollen en dit heeft ontsteking van andere delen van het gewricht tot gevolg. Als de aandoening verder gaat kunnen de botten die met het gewricht in verbinding staan zwak worden.

Meestal zijn er ook afwijkingen in het bloed :
- een verhoogde bloedbezinkingsnelheid,
- bloedarmoede,
- ijzertekort,
- (soms) positieve reumafactoren in het bloed.

Meer kans op osteoporose

RA is verbonden aan andere ziektes: De patiŽnten die door RA getroffen worden lopen een verhoogd risico op een zwakke beenderdichtheid of osteoporose, waardoor het risico op breuken verhoogt. Er bestaat ook een verhoogde kans op hart- en vaatziekten.

Specifieke oorzaken

kristalartritis: ten gevolge van kristallisatie van bv. uraat (jicht), calciumpyrofosfaat, hydroxyapatiet of oxalaat. Deze kristallen zetten zich af, waarna een reactie volgt vanuit het gewricht, met als gevolg een ontsteking.

reactieve artritis: een steriele gewrichtsontsteking als complicatie van een infectie elders in het lichaam bv. in tractus digestivus: acuut reuma of colitis ulcerosa, of in tractus urogenitalis.

Het gevolg van een gewrichtsontsteking is beschadiging van het kraakbeen. Kraakbeen geneest slecht omdat er geen bloedvaten in lopen. Duurt een gewrichtsontsteking te lang, dan leidt dit tot een gewrichtsdestructie.

Welke factoren beÔnvloeden de kans op RA?

Afweersysteem gericht tegen het eigen lichaam
De oorzaak van RA is nog niet gevonden. De chronische gewrichtsontsteking wordt waarschijnlijk veroorzaakt door een voortdurende reactie van cellen van het afweersysteem (T-cellen) die gericht is tegen lichaamseigen bestanddelen. Deze reactie wordt misschien in gang gezet door structuren in onder andere virussen en bacteriŽn (Inman, 1991; Rook & McCulloch, 1992). Gewoonlijk zal zo’n reactie worden onderdrukt door regulatiemechanismen van het afweersysteem. Het falen van deze regulatiemechanismen kan leiden tot chronische gewrichtsontsteking. Er zijn sterke aanwijzingen dat erfelijke factoren een rol spelen bij het falen van regulatiemechanismen (Winchester & Gregersen, 1988; Deighton & Walker, 1991).

Mogelijke rol van hormonale factoren
RA komt meer voor bij vrouwen dan bij mannen. Dit doet vermoeden dat hormonale factoren een rol spelen, hoewel het biologische mechanisme nog niet duidelijk is. Misschien hebben het gebruik van orale anticonceptiva (de pil), het doormaken van een zwangerschap of andere factoren die samenhangen met de voortplanting en seksueel gedrag een preventief effect op het ontstaan van RA of een verzachtend effect op het beloop ervan.

Artritis Psoriatica: er is hoop!

Het doel van dit verhaal is zoveel mogelijk mensen te bereiken die dagelijks lijden van pijn aan hun vingers, knieŽn enz. als een gevolg van de ziekte Artritis Psoriatica (AP). In de meeste teksten die ik gelezen heb is de hoofdboodschap: je moet leren leven met de pijn en de beste wapens tegen AP zijn medicijnen!

De boodschap die ik in dit verhaal wil brengen is: er is een goede kans dat als jij AP hebt je leven dramatisch kan verbeteren, niet door het nemen van medicijnen of het volgen van vage alternatieve therapieŽn, maar door simpelweg een aantal dingen niet te eten ! Het kost je dus niets en bovendien zul je geen beschadiging aan ťťn van je organen oplopen.

Alhoewel ik geen dokter ben (maar wiskundige aan de Radboud Universiteit in Nijmegen) denk ik dat wat ik meegemaakt heb met mijn vrouw, die ook AP heeft, zeker de moeite waard is om te lezen. Ik hoop dan ook dat er in de nabije toekomst wetenschappelijk onderzoek gedaan zal worden om de ervaringen die ik hieronder zal beschrijven, beter te begrijpen.

Experimental Gene Therapy “Abolishes” Arthritis Pain and Lessens Joint Damage

Early-stage research has found that a new gene therapy can nearly eliminate arthritis pain, and significantly reduce long-term damage to the affected joints, according to a study published today in the journal Arthritis and Rheumatism. While the study was done in mice, they are the first genetically engineered to develop osteoarthritis like humans, with the same genetic predisposition that makes some more likely to develop the disease, the authors said. If all goes well with a follow-up study currently underway, researchers will apply to the U.S. Food and Drug Administration for permission to begin human trials next year.

A newly identified immunosuppressive protein in rheumatoid arthritis

A complex autoimmune disease, rheumatoid arthritis (RA) is characterized by chronic inflammation and progressive joint damage. This process begins with hyperplasia, or excessive increase in size and thickness, of synovial tissue. Along with provoking cartilage and bone destruction, this abnormal tissue growth is resistant to apoptosis, the natural cell death vital to the generation of healthy new cells. Decoy receptor 3 (DcR3) is a newly identified member of the tumor necrosis factor receptor (TNFR) superfamily. A soluble protein, it is overexpressed in tumor cells, including lung and colon cancers, gastrointestinal tract tumors, and leukemia. It is also expressed in a variety of normal tissue—the colon, lung, stomach, spleen, lymph node, pancreas, and spinal cord. Because rheumatoid synovial cells share traits with tumor cells—both are resistant to apoptosis, both proliferate aggressively— DcR3 might play a role in the destructive course of RA. To investigate this possibility, researchers at Kobe University School of Medicine in Japan conducted the first study of DcR3 expression in RA fibroblast-like synoviocytes (FSL)—cells in the synovial membrane instrumental to the production of cartilage as well as synovial fluid. Increased alcohol intake associated with decreased risk of developing rheumatoid arthritis

New data presented today at EULAR 2007 suggest that alcohol may protect against rheumatoid arthritis, with three units a week exhibiting protective effects and ten units a week being more protective still. An alcohol consumption of three units per week or more also reduced the risk by smoking or by a genetic predisposition to RA.

Smoking, low levels of education and glucose tolerance all increase risks of developing rheumatoid arthritis

New data presented today at EULAR 2007, the Annual European Congress of Rheumatology in Barcelona, Spain, sheds light on the role of environmental and genetic risk factors in the development of rheumatoid arthritis (RA). Longer term breast feeding protects mother from risk of developing rheumatoid arthritis

Breast feeding for a period of 13 months or more has been shown to reduce the mother's the risk of developing rheumatoid arthritis, according to new data presented today at EULAR 2007, the Annual European Congress of Rheumatology in Barcelona, Spain. In the study, the longer the breast feeding period, the lower the mother's risk of developing RA in later life. Comparable use of oral contraceptives or hormone replacement therapy did not show a significant effect on the risk of developing RA.

Green tea compound may be a therapy for people with rheumatoid arthritis, University of Michigan study finds

A new study from the University of Michigan Health System suggests that a compound in green tea may provide therapeutic benefits to people with rheumatoid arthritis. The study, presented April 29 at the Experimental Biology 2007 in Washington, D.C., looks at a potent anti-inflammatory compound derived from green tea. Researchers found that the compound – called epigallocatechin-3-gallate (EGCG) – inhibited the production of several molecules in the immune system that contribute to inflammation and joint damage in people with rheumatoid arthritis. The compound from green tea also was found to suppress the inflammatory products in the connective tissue of people with rheumatoid arthritis. “Our research is a very promising step in the search for therapies for the joint destruction experienced by people who have rheumatoid arthritis,” says Salah-uddin Ahmed, Ph.D., lead researcher on the study.

Bing Cherries' Anti-Inflammatory Effects Documented

Bing cherries may help fight the inflammation of arthritis, heart disease and cancer, ARS scientists and their colleagues report in the April 2006 issue of the Journal of Nutrition (volume 136, pages 981 to 986). Investigators at ARS’ Western Human Nutrition Research Center, Davis, Calif., led the research, which was based on tests of 18 healthy men and women volunteers, aged 45 to 61, who ate a total of about 45 fresh Bing cherries throughout the day for 28 consecutive days.

Selenium and arthritis
Surveys indicate that individuals with rheumatoid arthritis, a chronic disease that causes pain, stiffness, swelling, and loss of function in joints, have reduced selenium levels in their blood. In addition, some individuals with arthritis have a low selenium intake.

The body's immune system naturally makes free radicals that can help destroy invading organisms and damaged tissue, but that can also harm healthy tissue . Selenium, as an antioxidant, may help to relieve symptoms of arthritis by controlling levels of free radicals. Current findings are considered preliminary, and further research is needed before selenium supplements can be recommended for individuals with arthritis.
