Regelmatig barstende koppijn ? Maar
liefst 700.000 Nederlanders leiden aan migraine. De Nederlandse onderzoeker Marco Post
zegt dat er bij een groot deel van die mensen een gaatje zit in een hartklep. De
gevestigde orde mort en bromt er natuurlijk weer over maar de onderzoeker krijgt bijval
uit Engeland van de duik dokter Dr Peter Wilmshurst. Deze onderzocht duikers die een gat
in het hart hadden opgelopen tijdens decompressie ongevallen (veroorzaakt door
stikstofbellen).
Ook is er een apparaat verschenen dat bij
sommige mensen die klachten zou kunnen verminderen, er is een onderzoek naar gedaan door
een Nederlandse neuroloog, deze studie is ook aangemeldt bij de Amerikaanse overheid als
clinical trial.

Ruim 700.000 migrainepatiënten
slikken foute medicijnen
Meer dan 700.000 Nederlandse
migrainepatiënten krijgen niet de medicijnen die voor hun aandoening op de markt zijn.
Van de twee miljoen Nederlandse migrainepatiënten zou de helft, één miljoen, baat
hebben bij zogenaamde triptanen, speciale antimigrainemiddelen. Helaas krijgen nog maar
203.000 migrainepatiënten deze medicijnen voorgeschreven; driekwart van de Nederlanders
koopt zelf huis- tuin- en keukenmiddeltjes die niet helpen. Uit onderzoek blijkt dat
migrainepatiënten meer dan drie jaar wachten voordat ze voor het eerst naar de dokter
gaan, om vervolgens afgescheept te worden met gewone pijnstillers.
De Nederlandse Vereniging van
Hoofdpijnpatiënten bepleit dat de nieuwe huisartsenstandaard op dit punt wordt aangepast.
De ernst van een zware migraineaanval wordt vergeleken met de amputatie van vier
ledematen. Op de lijst van de WHO staat migraine bij vrouwen op de 12e plaats en bij
mannen op de 19e plaats van ernstige ziektes. Sommige huisartsen die bij patiënten
geroepen worden, die midden in een migraineaanval zitten, bellen ofwel een ambulance of
geven een morfine-injectie. Zien zij de patiënt echter zonder aanval, op hun spreekuur,
dan schrijven zij paracetamol of ibuprofen voor.
De Nederlandse Vereniging van
Hoofdpijnpatiënten is geschokt over de recente onrust onder migrainepatiënten, die
massaal naar de cardioloog willen om eventuele gaatjes in hun hart te laten dichten. Dit
naar aanleiding van het, begin maart, verschenen proefschrift van Marco Post in het UMC te
Utrecht. Een ingreep die niet zonder risico is en waarvan het succes voor
migrainepatiënten nog niet bewezen is. Dit illustreert de wanhoop van de Nederlandse
migrainepatiënten. Goed werkende medicijnen zijn voor hen beschikbaar, maar worden veel
te weinig voorgeschreven

Migraine met of zonder aura
Prodromi : vóór een aanval vertonen
bepaalde migrainepatiënten voortekenen of prodromi. Deze tekenen variëren van patiënt
tot patiënt, maar vaak vertoont eenzelfde patiënt dezelfde prodromi bij verschillende
aanvallen. Dankzij deze tekenen, die worden waargenomen door de patiënt zelf of door zijn
omgeving, kan in dit stadium een efficiënte behandeling worden ingesteld. Als de
behandeling later wordt ingesteld, wordt het moeilijker de migraineaanval met aura onder
controle te krijgen.
Aura kan worden gedefinieerd als een
reeks neurologische stoornissen die optreden in het begin van een migraineaanval met aura.
Deze stoornissen zijn wellicht te wijten aan een verminderde irrigatie of het verlaagd
metabolisme in een deel van de hersenen. Deze stoornissen zijn reversibel, d.w.z. dat ze
niet blijven bestaan na de aanval, en verdwijnen zonder letsels achter te laten. Deze
stoornissen duren gewoonlijk minder dan 1 uur.

Kleine operatie kan
migraineprobleem oplossen
De mogelijkheid om migraine te verhelpen
door een gaatje tussen de boezems van het hart te sluiten, wordt international serieus
onderzocht. De Britse cardioloog Peter Wilmshurst verricht de eerste studie naar het
verband tussen een lichte hartafwijking en migraine.
„Als we migraine kunnen aanpakken
met een eenvoudige operatie die minder dan een uur vergt, is dat een reuze belangrijke
stap voor de patiënten", zei Wilmshurst maandag in Atlanta, waar meer dan 30.000
cardiologen bijeen zijn voor de jaarlijkse bijeenkomst van het Amerikaanse College van
Cardiologen.

Migraid device
Dit apparaat zou de klachten kunnen
verminderen, er is ook een klinische studie naar gedaan door neuroloog J.A.M. Kuster.

results. The aim of the study was to establish if it was possible to prevent migraine
headache following an aura from occurring. This was successful in 12% of the attacks
treated with the Migraid and 1.6% for the untreated attacks. Eighty percent of the
patients treated with the Migraid remained headache free for the rest of the day.

Permanent cure for migraine
A tiny umbrella implanted in the heart may be a remarkable new cure for migraine. The
device is usually used for holes in the heart to stop tiny blood clots and chemicals
escaping from the heart to the brain, where they may cause a stroke.
But doctors have discovered that patients who also suffered migraines experienced a
dramatic drop in the number of attacks after being fitted with the device.
"We believe 30 to 40 per cent of
migraines with aura are associated with a hole in the heart," says consultant
cardiologist Peter Wilmshurst of the Royal Shrewsbury Hospital, who was one of the first
to identify the link.
"We found that if we close the hole,
about half of patients are cured of migraine, and half get fewer attacks."

and the presence of right-to-left shunts has been reported in two studies. Right-to-left
shunts are also associated with some forms of decompression illness. While conducting
research in divers with decompression illness, it was our impression that divers with a
large shunt often had a history of migraine with aura in everyday life and after dives.

Provocative results of retrospective
studies prompt calls for randomized controlled trials.
(BETHESDA, MD)—Migraine headache
patients reported the painful attacks eased or even vanished after they underwent
procedures to close abnormalities that allow blood to flow between the atria of their
hearts, according to the results of two new studies in the Feb. 15, 2005 issue of the
Journal of the American College of Cardiology.
“What we observed is that after
closing the opening between heart atria, there was a dramatic reduction in the incidence
of migraine headaches in those patients who had complained of headaches prior to the
inter-atrial closure procedure,” said Jonathan Tobis at the University of California
in Los Angeles.
Dr. Tobis and his colleagues, including
lead author Babak Azarbal, M.D., reviewed 89 cases in which a device was threaded through
a catheter into an abnormal intra-atrial opening. Almost half the patients (37 of 89)
reported suffering migraine headaches before the procedure. By contrast, only 12 percent
of the general population reports suffering migraines. Three months after the procedure,
three-quarters of the migraine sufferers (28 of 37) reported their migraines were gone or
significantly improved.

and Migraine Headaches
Transcatheter closure of PFO or ASD
results in complete resolution of MHA in 60% of patients (75% of patients with migraine
and aura) and improvement in symptoms in 40% of the remaining patients. Interatrial
communications may play a role in the etiology of MHA either through paradoxic embolism or
humoral factors that escape degradation in bypassing the pulmonary circulation. A
randomized trial is needed to determine whether transcatheter

Migraine - New Study Supports
Importance Of Considering Gastric Stasis
Data from a newly published study in the
current issue of Headache indicates that migraine sufferers experience gastric stasis, or
the delayed emptying of the stomach, both during and outside of a migraine attack, and
that contrary to previous belief, gastric stasis did not worsen during an acute attack.
This data may suggest that migraine sufferers arephysiologically different in this regard
than non-migraine sufferers. In addition, the data may also yield key clinical insights as
the presence of gastric stasis, a physiologic phenomenon previously thought to only occur
in migraine patients during an acute attack, may interfere with patients getting fast pain
relief.

Acupuncture cuts tension headache
rates by almost half
Acupuncture in patients with tension-type
headache: randomised controlled trial BMJ Online First. Acupuncture is an effective
treatment for tension headache, cutting rates for sufferers by almost half, shows a study
on bmj.com this week. And a minimal acupuncture course works almost as well as traditional
Chinese therapy, say the researchers. In a randomised controlled trial - the gold standard
of clinical trials - researchers in Germany divided 270 patients with a similar severity
of tension headache into three groups.

Herbal extract effective in
preventing migraine
International Team of Researchers Find
Herbal Extract to be Effective In Preventing Migraine - An herbal extract offers
considerable help in preventing migraine headaches, according to an international research
team led by Dr. Richard B. Lipton of the Albert Einstein College of Medicine of Yeshiva
University. The extract comes from the petasites hybridus root (also known as butterbur),
which has been used for medicinal purposes since ancient times. The researchers reported
their findings in the December 28th issue of the journal Neurology.
“Butterbur is a traditional herbal
treatment for migraine prevention,” says Dr. Lipton, who is vice chair and professor
of neurology at Einstein. “Our study shows that butterbur really does reduce the
frequency of migraine attacks, so it's a welcome addition to the therapeutic arsenal we
have available to combat migraine.”
